Use this Personal Details form to record the essential personal details you require for your employees, including bank details and contact details for someone in the event of an emergency.
The form reminds individuals that they are required to complete a form when any of their personal details change.
It also includes an employee privacy notice, compliant with GDPR which explains: why you are gathering the personal data; what personal data you will hold in the course of employment; your legal basis for processing; what you will do with it; and how long you will keep different types of data for. It states employees rights under GDPR and also contains options should you need to include data sharing and the transfer of data outside of the European Economic Area (EEA).
You will also need to obtain a P45 or a completed HMRC New Starter Checklist Form from new employees.
