This Paternity Leave Request Form is an essential aid to making sure that both employees and employers understand the eligibility and options for taking paternity leave.
The form provides a reminder of the paperwork required to be attached should the employee be adopting.
Options to select in relation to the paternity leave start date are included: the expected date of birth; the actual date of birth; the date of adoption placement; the date the child arrives in the UK; a number of days/weeks after the birth/placement etc.
Paternity leave can only be taken within 56 days of the birth or placement and in blocks of 1 whole week, or 2 whole weeks, not odd days.
