2a. Partner/Father Ante-natal Appointment Request Form
This form is for employees to request time off work to accompany expectant mothers to ante-natal appointments.
The HR template serves as an ante-natal appointment attendance request form and also as a signed declaration of eligibility. An employee should not be asked for evidence of the appointment when accompanying an expectant mother, because such documentation is the property of the expectant mother.
Eligible employees have a statutory right to take unpaid time off from work to attend two ante-natal appointments with their partner, or the mother of their expected child.
Eligible partners/fathers are entitled to be absent from work for up to a maximum of 6.5 hours per ante-natal appointment, should this be necessary. This 6.5 hours includes travel time.
No minimum qualifying period of service is required to have the right to take unpaid time off work to attend up to two ante-natal appointments.
